Reporting notifiable diseases
There are more than 70 notifiable conditions in the ACT. The Chief Health Officer (CHO) must be informed of any known cases of these diseases under the Public Health Act 1997.
Doctors, authorised nurse practitioners, pathologists and hospital managers must notify the CHO of patients who they believe has or may have a notifiable disease.
How to report

To report any cases, download the report of notifiable disease or related death form.
Cases of some conditions, marked with a telephone icon on the form, should be notified by calling 02 5124 9213.
You can either:

- or fax it to 02 5124 8810.
