Transitional Therapy and Care Program

The Transitional Therapy and Care Program (TTCP) is a community-based program for older adults. It provides up to 12 weeks support and therapy to help improve functional capacity and independence after a hospital stay.

Our team consists of physiotherapists, occupational therapists, allied health assistants, social workers, speech pathologists dieticians, care support staff and case managers. You can access the service at our 15-bed residential facility or in your own home.

Eligibility for our service involves an assessment by the Aged Care Assessment Team (ACAT). You must also meet the following criteria:

  • be an older person or an Aboriginal and Torres Strait Islander person over the age of 50
  • be a public or private hospital inpatient at the time of assessment
  • have completed your episode of care and are medically stable
  • have the capacity to benefit from a goal-orientated and time limited program

To use our service, your inpatient team must refer you to the ACAT through My Aged Care.
