DAISY: Directory of Assistance and Information Services for You at the end of life

Daisy is an ACT based online directory of services and resource for patients, family and carers, as well as health professionals.  This database has been developed and is maintained by Palliative Care ACT. 

CareSearch

CareSearch is an online resource for healthcare providers, researchers and palliative specialists. It provides information and resources for palliative care.

Clare Holland House

Clare Holland House provides specialist palliative care services. Please contact Clare Holland House on 02 6273 0336 to discuss patient care needs.

You can also access referral forms for home based and inpatient care.

Palliative Care flipchart

The Palliative Care flipchart is a resource booklet that aims to encourage general practitioners and nursing staff to support older people in aged care facilities. It helps them recognise the desire to die with dignity in the home or residential aged care facility.

Program of Experience in the Palliative Approach (PEPA)

The Program of Experience in the Palliative Approach (PEPA) provides palliative care education for all healthcare professionals.

The aim of the program is to improve the quality and accessibility of palliative care services for all people with life-limiting conditions and their families.

The program is an initiative of the Australian Government Department of Heath and Ageing (DOHA) and is funded through the National Palliative Care Program.

PEPA in the ACT

PEPA in the ACT provides you with palliative care information, education, mentoring and support from the PEPA Manager and staff at Clare Holland House.

The placement also gives you experience in the Inpatient Unit and Home Based Palliative Care (HBPC) Service, an overview of the palliative care within The Canberra Hospital (TCH) and participation in a palliative approach workshop.

PEPA also conducts various workshops throughout the year that include topics such as grief and loss and spirituality and holistic care. PEPA also conducts specific GP workshops in collaboration with the ACT Division of General Practice.
