Hospital referrals
Health care workers from the ACT and surrounding NSW region can refer patients to our services.
We have included information for referrers on each relevant service and clinic page.
HealthPathways provides guidance for assessing and managing a patient with a particular symptom or condition. It also includes information about how to make a request to services in the local health system.
SmartForms is also available for referrers with eligible clinical systems (Medical Director, Genie Solutions and Best Practice) to submit electronic referrals for their patients online for outpatient and community services.

Community based referrals
You can contact our Central Health Intake (CHI) team to access a wide range of community based services.

Referral forms
All referral and intake forms can be faxed to (02) 5124 1082.
The Patient Information and Referral Form (PDF, 217.96 KB) needs to be completed for all referrals to CHI. Please ensure all relevant sections are completed, particularly demographic and funding information. For eNote users, please use the Patient Information and Referral Form - eNote (DOCX, 53.87 KB).
Some referrals require additional referral documentation. Please find this information below.
Medication administration
The Medical Officer Orders for Medication Administration (PDF, 78.87 KB) must be signed by a medical officer and use date format day/month/year. For eNote users, please use this Medical Officer Orders for Medication Administration - eNote (DOCX, 28.19 KB).
The first dose must be given by a medical officer. Please provide details of when this occurred.
The patient must supply the medication. Please clearly note medication name, dose, route, frequency and the start and end date for medication administration using format day/month/year.

Wound dressing
The CHI treatment orders (PDF, 63.78 KB) must be signed by a medical officer and use date format day/month/year. For eNote users, please use the CHI Treatment Orders - eNote (DOCX, 25.36 KB).
Description example below:
- Clean with Normal Saline
- Apply Jelonet
- Cover with combine
- Support with Tubiform
- Change 3 times/week
- Review in plastics clinic on 1 week
Any interventions that involve a urinary catheter
The Medical Officer's Orders for Urinary Catheter Management (PDF, 169.18 KB) must be signed by a medical officer and use date format day/month/year. Please ensure all sections are completed.
Drain management
The CHI treatment orders (PDF, 63.78 KB) must be signed by a medical officer and use date format day/month/year. For eNote users, please use the CHI Treatment Orders - eNote (DOCX, 25.36 KB).
It should also include the following information:
- type of drain
- how frequently to monitor output e.g. monitor drain output daily
- when to remove drain e.g. remove drain when output <30 ml/day for 2 consecutive days or on a specific date.
Please note all community nursing referrals require 24 hours’ notice before starting the service.
Tresillian Queen Elizabeth II Family Centre (QEII)
