You can contact the Central Health Intake (CHI) on 02 5124 9977 to access a wide range of community-based ACT Health services. They are available Monday, Tuesday, Thursday and Friday between 8am and 5pm and Wednesday between 8am and 3:30pm. This excludes public holidays.

Referrals for the Commonwealth Home Support Program

What is Commonwealth Home Support Program (CHSP)? 

  • It offers entry level support to help older people to remain in their homes.
  • CHSP services include Community Care nursing, podiatry, occupational therapy and physiotherapy. 
  • These services are funded by the Commonwealth and details are available through the website My Aged Care (MAC) https://www.myagedcare.gov.au/ or by calling MAC contact centre on 1800 200 422 Monday to Friday 8am – 8pm and Saturday 10am – 2pm.

Who is eligible?

  • Aboriginal and Torres Strait Islander people aged 50 years and over
  • People aged 65 years and over

How do I refer?

Exceptions

  • If CHSP eligible patient requires care within 72 hours eg wound dressings, then submit MAC referral and also print and fax copy to CHI on 5124 1082 so interim care can be arranged.
