Rehabilitation Nurse Practitioner Clinics
The Rehabilitation Nurse Practitioner provides services to people in the ACT with complex neurological conditions such as spinal cord injury, brain injury and other neurological disabilities or diseases.
Consultations can be provided in a variety of settings, inpatient units, ACT Health Centres, Village Creek Centre and home visits. All consultations require an appointment and the location will be negotiated taking into account the type of consultation, transport needs and proximity of various locations to the patients home.
Appointments will be made based on urgency of the health problem – generally within one to two weeks from the time of referral.
Referrals and bookings
Referrals will be accepted from Medical Staff, General Practitioners and other Health Professionals. Patients may self refer to the Nurse Practitioner for health issues related to neurological conditions such as spinal cord injury, brain injury and neurological diseases such as multiple sclerosis.
It is not necessary to have a referral from your General Practitioner, however, the Nurse Practitioner will provide your GP with a written summary of consultations, the treatment plan and any follow up which is required.
