Nursing and allied health professionals in our Aged Care Assessment Team assess the needs of frail older people and help them to access support.

Who we care for

The Aged Care Assessment Team helps frail, older people aged over 65 years or Aboriginal and Torres Strait Islander people aged over 50 years.

How to access this service

To access this service, please call My Aged Care on 1800 200 422 between 8am and 8pm Monday to Friday or 10am to 2pm Saturday.

The ACT Aged Care Assessment Team only assesses those who live in the ACT and have care needs that cannot be met by other community or aged care services.

What to expect

A member of the Aged Care Assessment Team will visit you at home. If assessment is needed as part of your discharge from hospital, we will see you at one of the public or private hospitals within the ACT.

What to bring to your appointment

  • Medicare card
  • Any letters or correspondence from your General Practitioner
  • Health Care Card and/or concession card (if you have one)
  • List of medicines you are currently taking (or the boxes), including medicines you have bought without a prescription, such as herbal supplements and vitamins
