Provider Information

The Health Directorate provides co-ordination services for chronic disease patients. Central to this co-ordination is the management of the patient's care planning across their whole range of services with primary, acute and community care providers.

Our Chronic Disease Clinical Network (CDMCN) has been established to provide a forum for improved clinical co-ordination of patients.

How does a health provider link to Health Directorate for chronic disease services?

If you are a clinician , GP or health service provider with chronic disease patient(s), we welcome referrals to our services.
