Other people can provide feedback on behalf of the consumer. However, if the person providing the feedback is not the consumer, then that person will need to complete a "release of information form".

The Health Records (Privacy and Access) Act 1997 sets out the information protection principles, which regulate the way ACT Health collects, stores, uses, accesses, corrects and discloses personal and health information.
